Ambulance Call Report (ACR) Requests The Ambulance Call Report (ACR) is the document which our Paramedics prepare and compile for all patients requesting our Paramedic services here in the Kenora District. The Health Information Custodian for the Kenora District Services Board – Northwest EMS is the Director/Chief of EMS. The Chief oversees the collection, storage and dissemination of any and all personal health information with the assistance of the Deputy Chief of Professional Standards. If you would like to obtain a copy of your Ambulance Call Report or one of behalf of a client, please fill out one of the appropriate form below. Completed forms can be delivered to any one of our Ambulance bases or our head office in Dryden. You may fax to our head office at 807-223-6500 or preferably e-mailed to the Director/Chief of EMS at atickner@kdsb.on.ca

The more information you can provide us with on your request will assist us with locating the desired information.

We operate Paramedic Services across 100,000 sq km and in 9 communities, 365 days a year and your help in narrowing down the search is extremely important.

Also, please note that we are not the only Paramedic Service operating in the Kenora District:

  • Ornge operates two Air Ambulance Bases in the
  • Kenora District.
  • A Helicopter base in Kenora
  • A fixed wing base in Sioux Lookout
  • Naotkamegwanning EMS operates their Paramedic
  • service from Naotkamegwanning   First Nations  and
  • often provides coverage when our staffs are busy and
  • take most of their patients in to Kenora for treatment.

Please ensure you are familiar with which service provided care to you or your client/patient as that will help ensure you receive the documentation in a timely manner.

Please allow one to two weeks for your request to be processed. These requests will be done as time permits and after immediate operational needs.  If you do not hear back from us within this time, please contact us again to ensure your request has been received.

Once the request has been received and the ACR has been located, depending on the method of correspondence you indicated, we will notify you either by telephone or e-mail that it is ready for pick up or delivery.   

We are able to fax or e-mail these documents to you or you can pick them up in person at our main office at 211 Princess Street, Dryden Ontario.

If you are a Police Service or Coroner requesting information for an investigation

If you are requesting an ACR due to an investigation, please ensure you have completed the form below.

Please allow one to two weeks for your request to be processed. These requests will be done as time permits and after immediate operational needs.  If you do not hear back from us within this time, please contact us again to ensure your request has been received.

If you are WSIB, an Insurance Company or Lawyer Requesting ACR on Behalf of a Client

If you are requesting an ACR on behalf of your client, please ensure you have completed the form below and have your client sign and authorize the release of information.  

Without your client’s authorization and consent, we cannot release the personal health information.

Please allow one to two weeks for your request to be processed. These requests will be done as time permits and after immediate operational needs.  If you do not hear back from us within this time, please contact us again to ensure your request has been received.

Note: Under the Personal Health Information Protection Act.2004.c.3 (PHIPA), the Kenora District Services Board – Northwest EMS abides by the Act to ensure personal health information is collected, used and stored in a manner that protects the confidentiality of the individual. Any information collected is used solely to facilitate effective provision of health care.

Northwest EMS is responsible for providing emergency pre hospital care in the District of Kenora. We currently have 9 stations and 114 Primary Care Paramedics.
